Please put your birthday, birth time, and the city you were born in in the additional information section of this order.
You can add requested dates and times to the addition information of this order. Please note that adding your requested dates and times does not mean that you will automatically be scheduled then. I will confirm a date with you by setting one up through Google Calendar and sending it to you.
